O6-Methylguanine-DNA Methyltransferase (MGMT) is an important DNA repair protein involved in tumor cell resistance to the cytostatic activity of chemotherapeutic alkylating agents. This protein is also effective in protecting normal cells against the genotoxic and carcinogenic effects of DNA alkylation. The alkylating drug resistance is caused by MGMT's ability to remove DNA alkyl groups introduced in the O6 position of guanine. MGMT is expressed in highly variable amounts, depending upon the cell and tissue type, species, and cellular growth characteristics. MGMT activity varies among groups of tumors and within a particular type of tumor.

Cancer chemotherapeutic alkylating agents (e.g. BCNU, CCNU, DTIC, procarbazine, temozolomide), act by inducing formation of lethal cross-links at the O6-alkylguanine position in DNA. MGMT transfers alkyl adducts from the O6-position of guanine in DNA (prior to cross-link formation) to a cysteine residue in its own sequence, thereby restoring DNA to its intact state. This transfer inactivates the MGMT enzyme and is irreversible; hence the level of MGMT in a cell is directly proportional to the level of DNA-damage it can tolerate. Because MGMT repairs the potentially cytotoxic, antitumor lesions induced in DNA by alkylating agents, tumors with low or no levels of MGMT will predictably be responsive to such drug therapy. Conversely, tumors with high levels of MGMT will likely be drug resistant. In normal tissues, MGMT acts as a suppressor of mutation and carcinogenesis. Abnormally low levels of MGMT in normal individuals/tissues may be an early marker of susceptibility to cancer.
