In cardiac tissue brain natriuretic peptide (BNP) is synthesized as 134aa precursor (prepro-BNP), which is cleaved by proteases to form a 26aa "signal" peptide and a 108aa pro-BNP. Proteolytic digestion of pro-BNP results in formation of 76aa amino-terminal NT-proBNP and biologically active 32aa BNP hormone molecule. Both proBNP and NTproBNP circulate in human plasma and have been proposed as markers for early identification of left ventricular dysfunction as well as prognostic markers of possible cardiac complications at patients with heart failure.

This gene is a member of the natriuretic peptide family and encodes a secreted protein which functions as a cardiac hormone. The protein undergoes two cleavage events, one within the cell and a second after secretion into the blood. The protein's biological actions include natriuresis, diuresis, vasorelaxation, inhibition of renin and aldosterone secretion, and a key role in cardiovascular homeostasis. A high concentration of this protein in the bloodstream is indicative of heart failure. The protein also acts as an antimicrobial peptide with antibacterial and antifungal activity.
