Background/Introduction: PDL1 (Programmed Death Ligand-1 or CD274) is a co-inhibitory member of the B7/CD28 family. The immunoglobulin (Ig) V-like domain of PDL1 plays an essential role in the interaction with its receptor. The receptor for PDL1is PD1 (Programmed Death-1, CD279), which is a co-inhibitory receptor. Activation of this pathway down-regulates several T cell functions such as proliferation and production of effector cytokines (for example IL-2 and IFN-gamma). On the other hand, blockage of the PD1/PDL1 pathway results in enhanced T cell proliferative capacity which results in augmented acute GVHD (Graft-Versus-Host Disease). This pathway is emerging as a major regulator converting effector T cells into exhausted T-cells during chronic infection and inflammation. PDL1 is expressed by several cell types, including T cells. Its constitutive expression is primarily restricted to cells of myeloid- lineage such as macrophages and dendritic cells in both mice and humans. In other cell types such as lymphoid -lineage cells, endothelial and epithelial cells, PDL1 expression can be either induced or up-regulated by IFN-gamma and TNF-alpha. Naïve murine T cells constitutively express low levels of PDL1, whereas naïve human T cells do not, and both murine and human T cells express markedly increased levels of B7-H1 after activation. Activated T cells upregulated expression of PD1, and its interaction with PDL1 induces T cell anergy, exhaustion, and apoptosis.
